Position Summary: Assist and support individuals in carrying out their day-to-day life activities and goals according to their life plans and staff action plans. Maintain a personalized, home like environment within the residence or facility. Provide each individual supported the opportunity to be independent and productive according to their individual capabilities with opportunities for choice and physical and social integration into their community. Assist individuals in understanding and exercising their personal rights as
appropriate to each individual supported.
Report to: Residence Coordinator and/or Program Director
Pay Rate: $17.00 - $17.00 per hour
Essential Duties and Responsibilities:
A: Assist in providing basic services to individuals in the areas of personal care, self-care, recreational services, communication, behavioral intervention and all other habilitation services offered to individuals as determined by the life plans.
B: Assist in planning, coordinating, implementing and evaluating individual’s service goals and total residence/apartment functioning and goals.
C: Maintain accurate, up to date and complete records and reports.
D: Safely accompany individuals to and from programs, work, social and recreational activities, medical appointments, and other destinations, etc.
E: Comply with all agency standards, rules, practices, policies and procedures.
F: Help individuals maintain physical care, toileting, showers, teeth, grooming, dressing and good hygiene. Physical lifting may be required, including such duties as transferring individuals to and from wheelchairs to toilets, beds, vans, showering and other daily living routines. (Residential Program)
G: Keep bedrooms, clothing and personal possessions in good condition. Launder personal clothing and bedding with the assistance of individuals.
H: Prepare meals and perform kitchen tasks with the assistance of the individuals.
I: Perform cleaning tasks (kitchen, dining room, living room, bedroom, bathroom and general areas) with the assistance of the individuals to the maximum degree their abilities allow. Follow all Infection Control procedures as per agency policy.
J: Shop for food, household supplies and clothing with the participation of the individuals to the maximum degree their abilities allow.
K: Prepare/complete written reports and documentation such as daily log, activity sheets, data collection, goal/habilitation sheets, monthly summaries, incident reports and other documentation as required.
L: Support individuals by taking them to community stores, places of worship, recreation and health facilities ensuring their safety while helping them to become a productive member of the community.
M: Participate as a member of a Treatment Team in the development, implementation and evaluation of the individual's Life Plan/Staff Action Plan.
N: Respond to crisis situations and medical emergencies. Summon police, fire, ambulance or other emergency personnel, if deemed necessary.
O: Carry out fire drills, evacuations and emergency procedures.
P: Attend staff meetings and in-service training sessions.
Q: Perform other duties and responsibilities as may be required or assigned by supervisor and/or administration.
Education and/or Experience: High School Diploma or equivalent. Valid N.Y. State Driver’s License required for Day Services Program**. Background working with developmental and disabled population preferred.
Special Requirements: Current Certifications in: CPR/First Aid, *Strategies for Crisis intervention and Prevention certification (SCIP) will be required as well as annual SCIP recertification.
*SCIP Certification and annual recertification is required in Day Services Programs.
Working Conditions: Flexibility of scheduling is required in order to meet individual supported and staffing needs. Apartment setting or private home environment where individuals receive supports for daily routines through supervision. Occasionally stressful environment due to challenging behavior(s) on the part of some individuals. Therefore, staff must be able to maintain calm presence in the face of verbal, physical and emotional challenges from individuals. Staff may also be exposed to individuals who may be
aggressive and carry communicable diseases.
