SDC realizes the importance of keeping families of developmental and intellectual disabilities united through a concerted effort and by doing so provide community habilitation services. The community habilitation program is a Medicaid Wavier Program that provide individuals with developmental and intellectual disabilities who reside at home an opportunity to learn and develop a variety of life skills with the guidance of a Direct Support Professional (DSP)/Comm Hab worker (CH). The staff are trained and provided the necessary tools to assist each individual in partnership with the families. Every individual come with their own set of skills and need to develop them. With the assistance of the Comm Hab worker the individual can achieve their maximum potential. The great part of this program is how everyone that is part of the individual's life can benefit. The individual's family members, the Medicaid Service Coordinator, the Program Coordinator and the individual can formulate a life plan. The Comm Hab worker provides that personalized one-on-one training while allowing the families to see the growth through the entire process. Furthermore, the individual and the Comm Hab worker explore the community utilizing public transportation, the library, the gym and other places of interest.
