Medicaid Service Coordination


Medicaid Service Coordination (MSC) is a Medicaid funded program that helps connect individuals and families to services. SDC believes in a person-centered approach, where the individual is at the center of the planning process, working with a Medicaid Service Coordinator of their choice in the development and implementation of their plan. SDC takes an objective approach to connect individuals and families with services and providers that would best serve their wants and needs. At this time, SDC offers Medicaid Service Coordination to those living in Manhattan or the Bronx and will work with individuals in obtaining Medicaid.

Some tasks that MSC’s perform include:

  • Educating individuals and/or families on services available
  • Ensuring that individuals’ rights are known and respected at all times
  • Facilitating in development of an individualized plan
  • Helping in the navigation of the service system
  • Monitoring current services to ensure quality provision
  • Assisting in Department of Education issues when applicable
  • Maintenancing of benefits such as Medicaid and SSI

Examples of services available through OPWDD:

  • Community Habilitation
  • Day Habilitation
  • Vocational Services
  • Respite (After-School, Weekend, Overnight)
  • Residential Services
  • Family Support
